A sports physical, also called pre-participation physical examinations (PPE), sports physicals are required before participation in a sport.
A PPE helps to assure that the athlete is healthy and physically ready for their chosen activity such as baseball, football, cheer, soccer, track or any sanctioned activity. The purpose of the PPE is to help minimize the risk of injury while your child plays.
A sports physical exam is not intended to replace a comprehensive annual wellness exam and other screenings.

Per Georgia High School Association By-Law 1.41(c) and the new State of Georgia law, the “Preparticipation Physical Examination” form must be signed by an M.D., D.O., or by a Physician’s Assistant, or an Advance Practice Nurse who has been delegated that task by an M.D. or D.O. Alterations (edits) to this copyrighted document are not permitted. The doctor or doctor’s designee should print and then sign their name on the appropriate lines found on page 3 and page 4 of the physical evaluation form.
When obtaining a physical examination, individuals have the option to use the services of their personal healthcare provider, choose from the facilities listed below or any facility where an M.D., D.O., or by a Physician’s Assistant, or an Advance Practice Nurse who has been delegated that task by an M.D. or D.O. practices medicine.
